He is talking to Kirk, in the presence of the Caretaker of the planet.
McCoy: I was thinking about a cabaret I know on Rigel II, and, uh, there were these two girls in a chorus line. And, well, here they are. Well, after all, I am on shore leave.

I really enjoyed the thinking behind this episode, that crews would NEED time off from their daily worries. And that sometimes you should be careful what you want.
Winner! And I appreciated the...variety...in McCoy's imagination.
I must confess some surprise that Kirk would think of an old flame (one who doesn't seem much of a conversationalist, too, not that Kirk cares about that anyway) instead of green Orion slave girls.
